Eskaton Hazel Shirley Manor: 1 Full-Time Position – Days – Fluency in Mandarin and/or Cantonese is required for this position

The Service Coordinator is responsible for linking the older adults, especially those who are frail or at risk, and/or the disabled residents to the supportive services they need to continue living independently.

Service coordination means the activity of linking a project resident to needed supportive services or medical agencies in the general community. Additionally, the term may cover case management, both formal and informal, in which the service coordinator assesses service needs, determines eligibility for public services, and makes resource allocation recommendations.

Job Requirements Training and Experience 36 hours training in service coordination. Must complete within 12 months of initial hiring or before (documented.) Two years experience with progressive responsibility in case management functions. Continuing Education: 12 hours annually (documented.) Job Knowledge Demonstrated working knowledge of supportive services and other resources for senior citizens and non-elderly disabled in the area served by the project. Demonstrated ability to advocate, organize, problem-solve with proven results for the elderly and disabled served. Other: As of June 1st, 2014 Eskaton’s Tobacco/Smoke Free Campus Policy prohibits employees from smoking anywhere on Eskaton’s property.
Education Requirements A Bachelor of Social Work or degree in Gerontology, Psychology or Counseling is preferable; a college degree is fully acceptable. However, individuals without a degree, but with appropriate work experiences, may be hired.
